The final mission of HITMAN 3, now part of HITMAN World of Assassination, contains a hidden alternate ending that is easy to miss on a first playthrough. It appears in Untouchable, the Carpathian Mountains mission in Romania, during Agent 47's final confrontation with Arthur Edwards, also known as the Constant. The original version of this guide described two secret endings and called one of them canonical. That is not quite correct. Untouchable has three possible resolutions: kill Arthur Edwards, erase Edwards' memories with the Serum, or unlock the actual hidden ending by having Agent 47 inject himself. The self-injection ending is an Easter egg and is generally treated as non-canonical. It deliberately echoes the beginning of the original Hitman: Codename 47.

How the Ending of Untouchable Works

When Agent 47 reaches the front of the train, a cutscene begins with Arthur Edwards. Edwards shows 47 a syringe containing the Serum, a memory-erasing compound connected to the experiments performed on 47 in the past. The substance is based on the same memory-suppression technology associated with Dr. Ort-Meyer's work. After the conversation ends, Edwards moves away and leaves the syringe behind. At this point, you have three options:
  1. Kill Arthur Edwards using a weapon or another method.
  2. Inject Arthur Edwards with the Serum and erase his memories.
  3. Wait with the Serum and inject Agent 47 himself to trigger the hidden ending.

How to Erase Arthur Edwards' Memory

Pick Up the Serum After the Cutscene
Pick Up the Serum After the Cutscene
After the cutscene, pick up the syringe from its case.If you approach Arthur Edwards while holding the Serum, you can inject him with it instead of killing him. Edwards resists, but 47 administers the drug and wipes his memories. Edwards is left confused and no longer appears to remember who he is or what has happened. Agent 47 then stops the train and leaves.
This is an alternate way to deal with Arthur Edwards, but it is not the hidden secret ending.
You can also simply kill Edwards. Either choice completes the main objective and leads to the regular ending sequence in which Providence is effectively dismantled and 47 ultimately reunites with Diana as partners.

How to Unlock the Secret Ending

To get the actual hidden ending, do not kill Arthur Edwards and do not inject him with the Serum.Follow these steps:
  1. Reach Arthur Edwards at the end of the Untouchable mission.
  2. Watch the conversation until control returns to Agent 47.
  3. Pick up the Serum from the case.
  4. Keep the syringe in your hand.
  5. Do nothing for approximately 47 seconds.
  6. Arthur Edwards will speak again and ask whether 47 is reconsidering his decision.
  7. A new interaction appears that allows Agent 47 to inject himself.
  8. Use the Serum on 47 and watch the hidden cutscene.
Some older guides describe the waiting period as one minute. Waiting that long also works, but the Easter egg is specifically built around the number 47, and the additional prompt appears after roughly 47 seconds.

What Happens in the Secret Ending?

After Agent 47 injects himself, the Serum begins erasing his memories. He loses consciousness while Arthur Edwards promises to take care of him. 47 later wakes up inside a padded room resembling the asylum environment from the beginning of Hitman: Codename 47. Arthur Edwards then speaks to him over the intercom using dialogue that deliberately mirrors Dr. Ort-Meyer's introduction from the first game. The implication is that 47 has effectively come full circle: instead of finally escaping the people who tried to control him, he has voluntarily surrendered his memories and returned to the role of a programmable assassin. This is why the scene works as an Easter egg rather than the canonical conclusion to the modern trilogy.

A New Father and Count Down From 47

Unlocking the hidden ending also completes the redacted A New Father Feat Challenge. You also receive the hidden achievement or trophy:
  • Count Down From 47 — "47 takes another nap."
The wording, the 47-second wait, and the final asylum scene are all deliberate references to Agent 47's history.

Which Ending Is Canon?

The hidden self-injection ending should not be treated as the canonical conclusion. The standard story resolution has 47 neutralize Arthur Edwards, either by killing him or erasing his memories, after which Providence collapses and the story continues with 47 and Diana working together again. The secret ending exists primarily as a callback for long-time Hitman fans and as an alternate "what if" conclusion.
So if you only want to see the secret ending, the important rule is simple: pick up the Serum, keep it in Agent 47's hand, wait 47 seconds, and use it on yourself instead of Arthur Edwards.
